- Individual cards of radiation monitorings, register of individual doses (in case of availability of own service of individual radiation control, in other case the agreement on conducting individual radiation control of personnel)
- The plan of carrying out radiation control (in case of availability of own service of radiation control, in other case the service provision agreement on production radiation supervision on works location with physical person or legal entity)
- Certificates of checking of devices of radiation control (in case of availability of own service of radiation control, in other case the service provision agreement on production radiation supervision on works location with physical person or legal entity)

Certification in the form of testing is undergone: 1) specialists to whose job responsibilities belong direct control of installation, ensuring nuclear, radiation, nuclear physical safety at activity implementation, in the field of use of atomic energy; 2) personnel of the nuclear facility to which job responsibilities accounting and control of nuclear materials, sources of ionizing radiation, radioactive waste, ensuring physical protection of the nuclear facility and nuclear materials belong 3) personnel of radiation, electrophysical installation to which job responsibilities belong control of radiation safety, accounting and control of sources of ionizing radiation Certification in the form of an interview there undergo the personnel of the nuclear, radiation, electrophysical facility to which job responsibilities belong control of radiation safety."

Step 5. Submission of documents on obtaining the license (personal presence is not required)

- The statement of natural person for the obtaining the license and (or) appendix to the license according to appendix 1 to this standard of public service in electronic form certified by the EDS of an uslugopoluchatel;
- The statement of the legal entity for the obtaining the license and (or) appendix to the license according to appendix 2 to this standard of public service in electronic form certified by the EDS of an uslugopoluchatel;
- The electronic copy of the document confirming payment in the budget of licensing fee for the right with occupation separate types of activity, except as specified payments through PShEP. At issue of appendices to the license licensing fee is not levied;
- Electronic copies of documents to activity in territories of the former test nuclear test sites and other territories polluted as a result of the conducted nuclear tests (further – electronic copies of documents) according to appendix 3 to this standard of public service;
